AGENT/BROKER RECRUITMENT
New
Agents without license must take an exam with the
Insurance Commission with the following requirements:
" Certificate of good
moral character issued by an official designated
by the insurance company and known to the Insurance
Commission or from any barangay official where
applicant resides.
" Two (2) identical
1x1 head photographs of the application. (recently
taken)
" Identification
Card
" Photocopy of naturalization
papers and original copy for verification (for
naturalized citizens of the Philippines).
" Sworn statement jointly
signed by the applicant and a company representative,
certifying that the applicant has been trained
in the kind of insurance contemplated in the license
desired. Such statement duly notarized by a notary
public.
Agents duly authorized
by the Insurance Commission:
" Application for
Insurance Agent's License
" CreCom Evaluation
Sheet
" Submit three (3)
1x1 & 2x2 ID size photograph (recently taken)
" Xerox copies of
Individual Income Tax returns together with the
Receipt of Payment, in any.
" Xerox copy of
Privilege Tax Receipt (as insurance agent) for
the current year, if the applicant has already
been issued a Certificate of Authority before.
" If the applicant is
a foreigner and a previous holder of a permanent
CA, xerox copies of his/her Alien Certificate
of Registration together with the corresponding
annual receipt (current).
" If the applicant
is a naturalized Filipino, xerox copies of his/her
naturalization papers.
" Certificate of
Authority Fee (charges include notarial and documentary
stamp fees)
PHP 280.00 for General Agents
PHP 180.00 for Special Agents, Unit Manager and
Ordinary Agents.
" Application letter
